Our staff is friendly and always ready to help with scheduling, insurance or anything else you may need during your time with us. For your convenience, we make the following information available:
Appointments:
Pain Consultants of West Florida looks forward to you joining our host of satisfied patients. Each of our doctors has a schedule tailored for your convenience. To schedule an appointment, please call us at 850-494-0000 or you can use our online Request an Appointment form.
If at any time you are unable to keep your appointment, please notify us at least 24 to 48 business hours in advance. We will be glad to reschedule your appointment at a more favorable time. We greatly appreciate your time and consideration and look forward to seeing you.
Patient Forms:
To help expedite your appointment with us, we have provided the following forms that you can print and fill out before coming in to see us:
- NEW PATIENT PACKET (complete set of forms)
- Health Insurance Portability and Accountability Act Authorization for Release of Information
- Pain Management Agreement
- Fall Prevention Balance and Dizziness Survey
Prescriptions:
Please call our office for all prescription refills ONE WEEK PRIOR to the end of your medication to ensure continuity of medication.
Insurance:
We accept most insurance plans. Please call us to verify whether or not we are participating with a particular insurance company.
Billing and Financial Responsibility:
Please review our financial policy or call our office for assistance.
